Request Appointment

If you require an appointment within the next 72 hours, please CALL our office to schedule – do not use this online form.  If it is more than 72 hours out, please indicate your preferred date(s) and time(s) and a scheduler will get back with you with availability. 

Please do not assume that “sending a request” automatically confirms your appointment.  Technology is inherently fallible, and our schedule is often quite full.  If you haven’t heard back from a scheduler within 24 hours, PLEASE call to confirm receipt and ensure your appointment is confirmed. 


Remember too that all airmen must have the MEDXPRESS confirmation number ready at check-in so we can get your exam downloaded and get you back timely.
